Home Blood Pressure Measurements

A one-page care plan for patients having Home Blood Pressure Measurements, covering Purpose, Expectations, Next Steps, Consistency, Indicators and Long-Term Success.

In this plan

  1. Purpose

    • I check my BP to guide my care. Normal is below 120 on top AND below 80 on bottom.
    • I use an automatic upper-arm monitor tested for accuracy, with a cuff that fits my arm.
    • I avoid caffeine, smoking, and exercise for 30 minutes before each check.
    • I empty my bladder, then rest quietly in a chair for 5 minutes. I do not talk or use my phone.
