   • I understand that insulin degludec is a long-acting insulin that controls my sugar between meals and overnight
   • I know my daily dose, blood sugar goals, and when my provider wants me to adjust or report my readings
   • I understand that this is my basal insulin and it does not replace rapid-acting insulin when that is also prescribed
   • I do not inject this insulin when my blood sugar is low, and I never change my dose without following my care plan
